I was there..

Did my first on call in UMMC yesterday. When i came in in the morning , I was surprised to find Elis's name on my list. I would have thought she would be done as an elective case.. but noo.. they had to put her in the emergency list. It was not a major Op as the papers reported , instead it was just a minor Ventriculoperitoneal (VP ) shunt.My Specialist on call assigned me to assist him in the OT. Haks.. talk about being involved with a "high profile" kid.

In the news..

I was glad that UMMC did not allow the cameras to air the surgery live. They followed her from the ward until the OT entrance. We received her and I'm not sure whether I was on air live then ..but I was wearing my mask.. so takde la nak cari glamour.. heks..

Anyway , Prof Vicky ( The Neuro surgeon) wanted minimal personnels in the OT.

" This is One shunt i can't afford to have it infected... "

Elis's problem was simple. * according to Prof Vicky laa..* She has a malformation of the ventricular system which causes her face disfigurement.An atypical hydrocephalus.In layman's term : The container of the Cerebro Spinal Fluid ( brain fluid ) seems to be not well developed causing it to fill in the space in between her eyes. Thus, causing the ugly big buldge that disfigured her nose . Her eyes position went lateral and that made her unable to close her eyes properly.The brain CT scan was interesting to look at as it showed a funny anatomy which was assymetrical.

As anaesthetists , we were worried if the disfigured anatomy affects the airway. Looking at her physique , it did not seem to be difficult. When we held mask on her face , Alhamdulillah there was no problem but it was a bit tricky as the fluctuating buldge on her nose was really dodgy. We didn't want to cause any "bursting" problems.
Intubation was a piece of cake.

What Prof Vicky did was just to divert the excess fluid from the brain into the abdominal cavity. A "bypass". It is an everyday and basic neurosurgery procedure and can be done anywhere if you're well trained. Prof Vicky was not keen on doing a major Op just yet - He wanted to see if this works and within minutes , the buldge dissapeared. haks.. It was a short procedure , but we discharged her to the ward late because we sort of overdosed her with morphine ! haks.. She remained stable but was a bit on the "high" side when we woke her up.

Emergency Department UMMC

Honestly , if you or your relatives are in an urgent dire-straits need of medical attention , Do not go to UMMC . I repeat.. DO NOT.. The best ? HKL.. you'll get your optimum , trained and of the highest consultation emergency care there... Only dire straits emergency yeah... please take note of that..

I was posted today in A&E . I was a replacement for a colleague who had to take MC . I guess with the amount of stress you have in ED here , its such a havoc job. Now i can understand why there are so many negative comments about the department. Its a classic example of first class facility but with THIRD CLASS mentality. If you drive along University road , one will be impress with the infrastructure ; so called DATARAN TRAUMA but inside.. Masya Allah... Ultimate chaos and i was in it this morning.

It's not as glamorous as ER or House where you have a team to work with, to discuss and to ensure the best medical care.I was in the YELLOW ZONE or fondly known as the ACM ( Acute Medical Care ) unit. Usually , us Anaesthetist will be in the RED ZONE area ; for emergency resuscitation but somehow i was pulled to do the yellow zone.

I was so surprised to find out that i was the only doctor in that area for today. Patients were non stop ; having problems ranging from Asthma to chest pain as well as severe abdominal pain. And i had to see and try to give my best care and consultation to each and everyone. Having to handle a 20 bedded emergency unit ALONE is not one's cup of tea. I was smirking caustically from the beginning to the end and my adrenaline was on its rush the whole day !! Obviously , my consultation was very sub optimal as i had to make fast decisions on what to do.. and i had no superior to consult . Damn man.. It was just about settling the immediate problem and at the same time glancing on the next patient.

The staff was wonderful and everybody worked to the best of their ability.Its just that we don't have enough staff .. apa lagi doctors !! Some patients who were not so urgent had to wait on their beds for quite some time. I just pity them but i tried my best to cater everybody's needs. Its just the bad HR planning by this department and imagine having a stand in doctor to be the main gatekeeper of the hospital for the whole day.Imagine , me the Anaesthesia MO with my limited year of experience ,untrained trying to manage the YELLOW ZONE of A&E alone.. I'm not trained to triage and fast diagnose.. Its not my major field and it is very UNFAIR to the patients !! The planning is really bad and from what i heard.. this has been the trend for years.. and nothing has changed. Everybody just HANTAM SAJALAH...

The last time I worked my ass off was as a houseman in P2. It really brought me the rush and memories of being so busy that you lose track of your time. Tau-tau jer.. from 8am.. dah pukul 2.. Damn.. and there were still patients coming in for treatment.The staff nurse had to stop me and forced me to take lunch in the pantry as she saw how i was literally on my toes since morning. Thank you S/N !!

Somehow , i enjoyed my experience today. ( Am I a masochist ??? pain is pleasure !! )However , I cannot imagine doing it day in day out.. gile mental stress !!!

Anyway , as I was about to leave , I saw a patient who was struggling while blood taking by my PM colleague. It was definitely a scene as she was screaming fretfully !!! Haks.. patut pun macam kenal . Guess who ? It was our dear SMTTDI English teacher , Cikgu Rohana. ( once , she was Ustaz's nemesis ; remember ?? ) . Of course she recognized me ; and the first thing she said was..

" Alamak !! Anak murid cikgu ke.. dah berapa lama jadik doktor ??!! Mafeitzeral.. Don't tell anybody in school about me in here today !! Malu lah cikgu !! " . Hahahaha..I just laughed and calmed her down.

My colleague failed in her attempts to take blood as she was a "difficult" patient. She has phobia with blood taking maybe because of her past experience . She was hyperventilating and very panicky !! I managed to take her required blood after such "high drama"!! I quickly wish her get well soon as she was pushed up to the ward.

Well.. that made my day.. *smile*

Surgery LIVE telecast

If you guys have been reading the papers , RTM will be airing live Elis Krisyanto Sari's surgery on Monday in UM. It's a major Op but somehow i still do not understand it's significance.

I'm on call on Monday,so tune in. haks.. Maybe you'll see me on TV then.. hahahaha.. gila glamour !!!

p/s I strongly suggest that the government airs live Lina Joy's Putrajaya Federal Court appeal on the 28th of July...

Italian hangover

I was zonked out yesterday. Watched the World Cup finals from 2 am and went to work straight away after that. I was in charge of daycare surgery thus the 8am sharp start.I was finished by noon but my pre med rounds took me hours to sort out.7 paediatrics cases man ..complicated lak tu. By the time i went home I was so stoned that i slept from about 4pm till 9pm !! haks.. gile miss sembahyang !!!

I was satisfied with the game , initially it promised goals as it was 1-1 by the 20th minute. As time progressed , you can sense that nobody will be scoring and thus the penultimate penalties. It was obvious from their body language ; whose team was more confident Besides Zidane's heroic head butt , there was nothing else exciting about the game. The italians deserved to win .. and now they are the most successful European team to win the World Cup. Forza Azzuri !!

I was quite glad this morning to find out that the surgeons cancelled 4 cases , making only 3 to do today. I was doing the CVL list ; surgeons putting on a permanent IV access on the child's major veins via surgery. This is usually common for terminally ill patients for their parenteral feeding or chemotherapy. This is also always a nightmare for us to anaesthetize ; imagine telling the parents about the risk of having their child expired after administrating anaesthesia.

Anyway , I was really sad for this 12 DAY OLD baby. She was very unfortunate to have her intestines resected on her 5th day of life ; she had duodenal atresia ; in layman's term ; her gut was not well formed therefore causing absolute obstruction. It was a very major Op then; the surgeons were hoping to save some of her gut ; but unfortunately it was really bad. Her parents did not have a clue about all of this ; their initial complaint was baby did not defecate for 4 days since birth.

SHe looked very thin ; cachexic and malnourished. Imagine , 12 days of life and she just cannot take feeds . The line inserted today was for her nourishment ; but honestly , her prognosis is very poor. There was no way she can absorb any kind of nutrition into her body and she will die slowly. The measure taken today was just palliative ; and nothing can actually be done. So sad...

I was with a final year student doing the list. She was a bit of a nightmare to start off with because we can't intubate her.. Well , we did but her tube dislodged when we repositioned her head and she desaturated to 48% with her pulse rate coming down fast. I went a bit panic because the staff nurse was extra "rajin" today by throwing away all my emergency drugs prepared especially for this baby girl ! haks.. Nasib baik my 1mg in 10ml atropine was around .. so we resuscitated her and she all right with LMA. Somehow , with the small ETT she has some obstruction.. somewhere..

The procedure was simple ; thus the short time of anaesthesia. Before sending her off to the Paeds ICU , I was a bit disturbed.. because i know her prognosis and maybe ..that'll be the last time we meet..

Super Superb..

We watched Superman Returns last night. We dropped baby Dzaeffran at his nenek's place and off we went to Cineleisure ; a new complex in Mutiara Damansara. The cinema was a bit secluded because only the cinema was operational on the 3rd floor. Otherwise , the complex is still in its refurbishment and no other retailers around.The complex is huge and this place will be a big hit to movie buffs ! At least , it would decongest GSC in One Utama and tickets for new movies will be easily available.

SUPERMAN was WONDERFUL !!! I fell in love with the character again last night . When the famous theme song was played at the beginning of the movie , you just had that rush ; the experience rekindled ; remember when we were young and excited with Superman ? Yes..its that feeling... The plot was Ok la.. a good connection of why there was no Superman movie for quite sometime. Bryan Singer is cunning with his ending and I bet ya.. everybody is waiting for the next instalment ; looking forward to Kal-El's son.. Happy Father's Day !!!!

Anyway , an interesting issue came up towards the end of the show. How do you resuscitate Superman ? hahahaha.. The airway was maintained with good ventilation bagging.. but they can't even poke adrenaline for Superman and it was funny when they tried to defibrillate him ! hahahaha... I guess , he maybe a difficult intubation case as his jaw maybe too stiff for us to laryngoscope. Nak inject pun tak boleh , macam mana nak pasang IV access for Sux ? hahahaha... I guess , this will be debated next week with my colleagues in determining his Super physiology!!

Italy vs ...

And the Italians had to score 2 minutes before extra time ended. The timing was very cruel for the whole of Germany who thought their country would be in the finals through penalties.I did not watch the whole match bas i slept straight away after badminton last night. Dzaeffran woke me up early this morning for his feeds and i just switched the tube on. Extra time 0-0. I just laughed my heart out when Italy scored thier crucial goal. Such timing... susah susah aje..

Frabce are up against the portuguese today. Seeing the way Zidane played the other nite , my money is on the French team. Do not rule out the sore tactics of Scolari , and who knows Portugal may win World Cup 2006 !!!!

A dissapointing World Cup .. so far..

Reaching out..


I stayed up late for the past 2 nights to watch the much awaited quarter finals between the super powers of world football. On paper , the games looked juicy and I was waiting for one of those classic world cup games. So much promise was given during the group stages where a century of goals scored and most of them so beautiful. I thought Germany 2006 would be the best World Cup ever...

I was wrong .. and my headache is really bad now.

Germany vs Argentina 0-0 : The Germans showed their mantle with their tradition of resilience and never say die attitude. They were lucky to win the shoot out as Argentina just fizzled out. After the Serbia 6-0 trashing, I had put my money on Argentina to win the Cup but somehow , their game , tactics were just crap in this game and so..they deserved the lost..

Italy vs Ukraine 3-0 : Ukraine is just too young to be a contender. Italy with ease outclassed Ukraine with the handsome 3-0 score.It was the finishing that made the difference , Ukraine are not world class enough compared to the Italians on the day of the unknowns making their country proud with the win.

England vs Portugal 0-0 : Pissed me off so much. The portuguese did not take advantage of England's pathetic display of football. Their tactics are just plain stupid and it is such a waste of Gerrard / Lampard / Joe Cole talent. It showed so much how low their self esteem were when they failed to convert their crucial shoot out. They failed miserably and i reckon the whole squad is depressed now.

Brazil vs France 0-1: Brazil were just waiting for to be kicked out and lucky France manage to oust them.France was not exactly the best team in the world but.. Brazil were just jaded. Ronaldo / Ronaldinho was invisible during the game and their creative flair was non existent. They looked lost and just did not attack well. Serves them right for dissapointing the fans all over the world.

What happened to classic world cup drama ? I really do hope that the semi finals will give something more.. or this World Cup will be just as boring as Italia 1990 or USA 1994...

Fishy day out

Aquaria..


Wifey wanted to have a day out with the baby and we decided on Aquaria at the very last minute. Dzaeffran seems to be more receptive now ; thus the decision to bring him to Malaysia's biggest aquarium to stimulate his mind. I guess , it was a mind blowing experience for him as he was so excited the whole 2 hours that he forgot his feeds !! He was so engrossed with the fishes that when we got home , he was so knackered that he slept soundly the entire night !!

The Aquaria concept is very similar to the Pulau Sentosa Underwaterworld that we visited last year. The setting up is very professional and of course , plenty of visitors yesterday. I did not bring my tripod ; therefore my attempts on single fish photography was unsuccessful due to the fish not sitting still !! haks.. obviously.. Unfortunately , during the tunnel ride , the amount of fishes is definitely less than those in Singapore. Don't worry , we'll catch up !!

It was an enjoyable outing. I would definitely recommend Aquaria to those who enjoys looking at beautiful fishes. I certainly enjoyed the 2 meter long stingray who ruled the waters here in Aquaria KLCC !!!

Haks.. saw his name on the list. I was not really sure if it was him ; then i met him during my pre anaesthesia rounds and confirmed..

Yup.. Rabuan Pit... the well known national athlete in the 80's.If you are still not sure , he was the malaysian who won gold unexpectedly in the 100m dash during the 1982 ASIAN GAMES.If you are still not sure.. then.. malu aahh jadi orang malaysia !! We seldom produce sprinters and he is one of our greats besides Dr M Jegathesan and the glamourous Watson Nyambek. He's 50 now and still looks youthful eventhough he has put on weight.

A loss

I was studyin.. ( well.. was on my study table trying really hard.. ) last night , waiting for the England game when i received an SMS from Papa. Tok Jah had peacefully passed away at about 930pm Sunday night after being ill for about 1 month in Kuantan. It abrupted my pathetic attempt to study and made preparations to go back to Kuantan. Wifey could not find a replacement for her call and I went back with Papa and Mama after Subuh early this morning.

I don't know Tok Jah really well but we would often visit her everytime we are in Kuantan. My father's life story is a bit complicated you see. She is my father's real mother. She was married to my real grandfather ; Atuk Salleh in Terengganu. I never really knew what happened , but soon after my father's birth he was taken care by Tok Jah's sister , mak tuk and the man i called Atuk Aki , arwah my grandfather who passed away 2 years ago. Atuk Salleh and Tok Jah got divorced and both married somebody else. Tok Jah was married to Arwah Tok Mubin and had 9 children with him. Atuk Salleh remarried twice ( I'm carrying his genes you know.. hahaha.. ) , and had LOADS of children. I don't really know the clan of Batu 6 in Kuala Terengganu. I never understood the complexity of the family tree until i finished school. I used to boast of having the most grandparents of all time !!

Honestly,I am very curious but I had never asked my father why Atuk Salleh and Tuk Jah separated ; maybe Papa don't know too.Will it be a sensitive question ? Papa was taken care well by Atuk Aki and Maktuk who coincidentally had fertility problems ; at the same time occasionally would still be in contact with Tok Jah who was in Kampung Padang. My father has always been respected as the eldest brother amongst the siblings of a different father. Uncle Samad / Husin / Hasan / Mat Kassim / Amir / Zul / Mak cik Sah / Nor and Che Mek Su ( who is a trisomy 21 ). We seldom go to Kuantan for Ist day of Raya ; and i could scarcely sense Papa's loneliness of exile when we were there. If I am not mistaken , 1992 was the first time we slept and celebrated Raya together in KAmpung Padang. We were more to PRO- MUAR rather then Pahang -Terengganu.

We arrived in Kampung Padang at about 9am. Papa drove quite fast and the highway certainly makes travelling to Kuantan more convenient.It was very busy - everybody was waiting in the lawn area.The kampung atmosphere was really alive here. Arwah Tok Mubin was somebody who was wealthy , and his land area is quite handsome. Its like a small complex with small houses (occupied by his children) surrounding the main block ; and a dedicated surau in the middle of the complex.

Tok Jah's body had already been cleaned and the "kapan" was set. It was time for all the siblings to come in and have their last look before the face is covered.Papa was given the honour to lead the pack - being the eldest. I could feel the sadness and as i looked at everybody's face. Uncle Samad to Muldip had tears running after kissing their beloved mum for the last time.It was interesting to see how they try to hide their tears by distracting themselves by hugging their babies.

We prayed for Tok Jah in the built in surau - it was led by Uncle Zul , one of my uncles who is an Ustaz by profession. It was certainly touching to see how Tok Jah's children independently organized the event just now ; with knowledge and confidence. I suppose every parent would want this trait in their children and yearning for Sallehs and Sollehahs after every prayers.

I could not hold back my tears during the Talqin. I suppose nobody could when hearing Uncle Zul reciting the holy words . He was full of emotion and love ; having to stop a couple of times . He stattered ; not because of the jitters as he had done it dozens of time but he was trying very hard to stop himself from crying out loud. It was amazing to see how all the men there just cried and felt the true feeling of love shown by her son.

That was the end to a plentiful life lived by Atuk Jah for 84 years. Dzaeffran will never know her personally ; maybe descriptively in the future when he reads this blog entry. It was an emotional day but that is what life is all about. How we live our life in this world is at god's will and when the time comes ; He will take back what belongs to him. Alfatihah to my dear nenek.. and Semoga rohnya dicucuri rahmat..

The long 9 hour surgery

I was involved in a long surgery with the ENT team yesterday. We kind of expected it , as the surgeons told us that it would take 7 hours and we estimated to add another 2. We were right. We started off at 12 pm and ended at about 9.30 pm with the patient extubated and responding well.

The day started in a panicky manner when we had problems with our first patient. A 72 year old chinese man who had bilateral enlarged infected tonsils with matted cervical lymph nodes. HIs airway looked ok on assessment ; and we were confident that we initiated nasal intubation. After induction , my senior gave the full dose of muscle relaxant and i laryngoscoped the patient. ALAMAK... The anatomy was distorted and i could hardly see the epiglottis.THe so called tonsils were blocking my view ; not to add the truama I caused when inserting the tube through the nose. Cormack Lehane IV tu !! Luckily his saturation was maintained at 99-100 % all the time ; showing us that it was more of a can't intubate can ventilate scenario. He tried a few times but was unsuccessful.

Called for help and Prof Lucy came to the rescue. It was indeed difficult and of course i had to take the rap ! Well , I've informed my senior colleagues and on their assessment did not anticipate this problem. We managed to intubate the patient and the Op went well. With the removal of his tonsils which were damn BIG .. I think it would be not that problematic to intubate him. But i suggest SUX should be the first drug to use..

Then we proceeded to the next case. 40 year old Malay man with a right parotid growth , a lump as big as a size 3 football ( YES IT WAS THAT BIG ! ) on his right side of the neck. Kind of reminded me of this one Malay actor. The NET team planned to excise the tumour and then re construct the neck using the chest muscle. ( something like the picture above ). I could not imagine if it was the plastic team who would take i reckon more than 24 hours to settle this. One thing though , we were lucky that there was no mass encroachment to the airway. But we were more careful this time ; we used SUX and we were OK this time around .COrmack Lehane 1.

We prepared the patient with all the invasive lines; My triple lumen was spot on on his left subclavian eventhough his size looked like a big challenge.We had problems ventilating him initially but managed to get good lung volumes after some time while altering his settings. The surgery started on time and went on..and on... and on.. and on..

The op was uneventful except for this one time when he desaturated to about 70%. The surgeons pressed on his long circuit connection and accidentally disconnected it. In the beginning I failed to bag the patient and he looked bad that i called for help. SOmehow , I managed to sort out the problem and when Vas and Yoga came in.. His saturation was already 99%.The Op was interesting and i should have brought my camera . Next time... next time..

It was long.. but not long enough as there will be times when surgery can go up to 24 hours. And being UM , there is no shift system ; so one has to stay on all the way till the end. Haks.. I'll write about it when i'm there.. don;t worry..

Trying again

I just can't sleep right now. Its going to be 3 am soon.Glanced through my notes and books on topics for tutorial tomorrow. It has been a very busy 2 weeks with the Profs giving tutorials like nobody's business. It's not really for us the freshies ; but having rare chances for sessions with the Profs.. why not ? On the other hand.. I have yet to decide on whether i would be watching England's game tonight.Is it worth it ?

Came back from badminton with the guys quite late after our ritual "drink after the game" in 222. Nowadays we are eating less but drinks are free flowing with generousity. takde alcohol pun tapi perangai macam orang mabuk.. hahahaha..

Wifey got a bit excited as her period is a bit late. She started to have back her menses last month. She is still breast feeding though which may make one's cycle to be a bit haywire. It was negative.. I'm not sure whether she's keen or not but i know she yearns for another baby. Kalau positive.. haks.. giller power siot ..baru berapa kali.. haks.. but i suppose.. not that POWER yet la .. eih ??

Steady state..

I'm still adapting to the system here in UMMC OT. Iti s a bit different from what I'm used to , not just the physical machines , but how you work , who you work with and establishing rapport with the supporting staff.

Honestly , the machines in our OT are real old and looks old .I'm not sure why they are still keeping the old ULCO ventilator machines. Ancient giller !! I will snap pictures for your viewing some find day. Takde budget ke UM ni ? haks.. HKL and Selayang has better reliable machines. I miss my old Datex Ohmeda in HKL !! haks..

Everyday , I'm doing list with either a senior of fellow colleague. We do our pre meds ; see patients but we don't have to report to any specialist unless it is really problematic ! haks.. Well, i suppose it is important to develop our confidence in preparing anaesthesia plan as a specialist soon. It felt awkward in the beginning but coping slowly.. For example , I was sent to do ECT list ( electroconvulsive therapy - zappin them up ) for psychiatric patients on my third day of life .. belasah jer lah.. and yesterday , I was guiding a fellow colleague who had never done ECT before.. haks.. macam tera je..

It is definitely more academic here in UM. When the Profs speak up during our CME or tutorial , you can appreciate their knowledge and see how they apply simple physiology and pharmaco in our daily practice. Very high expectations woo..Haks.. I'm impressed really.. coz it was never academic but more of practical clinical practice before this..

P/S An I guess only in UM , they have a surgical remedy for failure of ejaculation..

Rookie

It'll be my third day tomorrow in UMMC OT.Here I am in an institution whose status pronounces grandiousity and overwhelming. My impression.. well.. haks.. lets just say I'll reserve my comments till i graduate. Let me go through the system first safely.. and then will I be frank with my opinion. Besides that , I'm still new so let me grow and mature with what is present here. I did my first eye list on Monday.. and well..well.. I was shocked to see my ventilator !! haks.. I'll be doing the ECT list tomorrow which will be one of the light lists over here.Thank god for that.

We had our orientation last week ; met most of the Professors in the Anaesthesia department. They are certainly smart and that is why they are the professors !! haks.. For the first time in many years ; we will have 12 trainees at one time. Usually they'll have only 5 - 8 people in a batch in the first year. KKM was certainly kind this year providing extra trainees compared to the yesteryears. I don't know all of them yet that well but i reckon i will get to know them in my coming four years here..

By reputation , Anaesthesia in MU is very demanding and challenging at the same time. Horror stories about the working hours and the kind of job that you do is certainly true. The teaching staff here admits that as well and it makes you wonder more about your decision !! haks.. Looking at the bright side , I suppose it is better to go in with certain negative expectation so that you can prepare mentally to pull yourself through.When something goes wrong , you'll be prepared for the worst and maybe able to recover succesfully. It will depend on how strong one's will will be.. Alamak... kenapa macam ni ha ???

The passing rate is not encouraging but people have done it before.When you think about it , of course it;ll be more justified to have specialists to be right 80-90% about their actions and treatment of the time rather than the iffy 50-50 ones !! I;ve seen the specialists who were previously trained here and they are the kewl joes that i admire.. Hopefully i will be one of them in the near future.

Here goes eih... 4 years of roller coaster ride..

A budding rose on a different petal ..

UM - Dewan Jemerlang. 9.00 am 31st of May 2006.

Luckily , i put my logic cap on early this morning. I woke up and suddenly asked myself , where was this place ? I actually thought that we were suppose to assemble in the Postgraduate Centre. I did not ask anybody but just assumed. Hooked myself to streamyx at 7.15 am and searched via Google.

Laaaaa... dekat medical faculty rupanya ! Miles away from the place i thought it was suppose to be in. Parking was horrible though ; i had to walk quite a distance in the hot morning sun . I just had to wear all black with a white tie ( macam MAfia lah pulak..) - simply "brilliant" fashion sense isn't it.. haks...

Anyway , arrived just in time for UM's Dean of Medical Faculty introduction speech. There were about 100+ of us in the hall. Wow.. I'm actually moving on with time ; suppose to be linear in pursuing my "dream". Coincidentally , there were quite a number of familiar faces ; not just my peers from the UiTM fraternity , but individuals i met along my path so far.It was like a reunion meeting back colleagues from UK and Eire , to those during my IMU days and HKL friends. Everybody looking eager in beginning their next milestone of life or maybe putting one's leg to eternal agony and struggle !

What is the purpose really ? How sure am I in taking this step? Is it worth the gut and sweat to achieve titles of "GIST" or "IAN" ? ( N.B. Anaesthesiolo"GIST" , Paediatric"IAN" )

For money and wealth ? Well.. it IS a factor but i reckon it would be much simpler bridging in another path . Becoming a clinical specialist in anaesthesia does not gurantee cash rolling to you with a snap.

To enrich my knowledge ? Naahh.. I am not a keen academician. The training will only increase my experience and knowledge will compliment it indirectly. Reading is sufficient but how useful is it ? Only time will tell..

For the betterment of man ? Sounds corny and just shit crap . One may hope to do so but maybe only in one's utopic intention . We hope it have it in mind to do this job with sincerity and dignity.

A better life ? No life will be a better description in entering the Master programme. Will i miss baby Dzaeff growing whilst struggling to get all the info into my head ? Precious moments.. Without being in the programme itself I've had situations with wifey ; what more if one is in the programme ?

Life is about experimenting and making decisions.When an infant learns to roll , crawl and walk , they just do it and see what happens next. Nobody knows what is in store ; how things may come up or turn about.Life is never simple.Planning is one thing but the execution and the outcome may not be in the initial objective. It is suppose to create excitement and sense of achivement but it all depends on what we really want.

"Tawakkal" is the keyword said to bring a bit of calmness when one is in doubt. Lets just sit back , relax and "tawakkal" ..

The better team lost...

the chosen one..


I suppose Mourinho's bold statement would be justified in this game.. well , not really !!

I did not plan to watch the game in Bukit Jalil coz i did not have "kakis" to go with. Fortunately , Dil had an extra ticket so .. THANK YOU very much Dil for the wonderful evening !!( Thank you to your Chief as well lar.. haks..) It was definitely my first time seating in the VIP area which i felt a bit awkward in the beginning. Sitting among the aristocrats.. haks.. terasa wooh...Dil's parents were there too and we had our fair share of shouting and harrasing the national team !!

Myteam was definitely the underdogs and was not expected to shine being the "mee segera". However , the game was enjoyable as Myteam definitely made Ibrahim Saad to sweat in the first 70 minutes. Myteam was leading 1-0 and the fairy tale for the amateurs nearly became reality. Unfortunately , 2 goals from 2 set pieces broke their hearts and the mixed Malaysia team won the game.

Overall , the game lacked quality but was full of drama and suspense. Who would have thought Myteam able to score the early goal and made the Malaysian camp panicked.Myteam had the crowd's backing and performed their best .Everybody was worried when Myteam captain was sent off immediately after the first goal but sigh in relief when it became even with the marching orders for Zaquan of Malaysia for dangerous play. It was good to see Myteam's determination and effort ; but technically they fared badly. Malaysia did not have a natural finisher hence their frustration for not scoring.

Kudos to myteam and kudos to Malaysia for a respectable entertainment - just in time for the FIFA World Cup !!

farewell party


We had our farewell party last Friday. The food was good which completed our one week of free food !! ( We had regional anaesthesia workshop this week ). I felt very sad to leave the department . It was very thoughtful of them to do such a thing for us eventhough we are mere ikan bilis. Well , 11 of us will be going out - debiliating the department for a while before the new masters student will come to Selayang.

My gratitude to the department especially to my dear boss , Dr Arbayah. Thank you too to Dr HAshim , Dr Bakar , Dr Suresh , Dr Hanapi , Dr Cheah , Dr Ng and Dr Laila. You were all great and i appreciated your wisdom and knowledge that was passed to me..

KUALA LUMPUR 24 Mei - Seorang pensyarah universiti hari ini mencadangkan supaya kaum lelaki dibenarkan mengamalkan perkahwinan MISYAR bagi mengatasi masalah ramai wanita tidak berkahwin.

Malah menurut Pensyarah Akademi Pengajian Islam Universiti Malaya, Prof. Datuk Dr. Mahmud Zuhdi Abdul Majid, perkahwinan tersebut juga mampu mengurangkan perlakuan maksiat di dalam masyarakat.

Perkahwinan misyar bermaksud, suami tidak perlu memikul tanggungjawab memberi nafkah zahir seperti wang ringgit dan pakaian, tetapi CUKUP HANYA MEMENUHI NAFKAH BATIN TERHADAP ISTERI.

Perkahwinan seumpama ini banyak berlaku di kalangan wanita yang berkedudukan tinggi dan berpendapatan lumayan di negara-negara Arab.Beberapa ulama termasuk Dr. Yusuf al-Qaradawi dalam Fatwa Muasaraahnya berpendapat nikah misyar adalah sah.

``Jika pihak isteri bersetuju bahawa lelaki tidak perlu memberi nafkah zahir seperti menyediakan rumah, pakaian dan sebagainya, tetapi hanya bertanggungjawab terhadap nafkah batin saja, maka ia tidak menjadi masalah,'' tegasnya.

``Sekiranya tidak timbul soal siapa yang patut memberi nafkah atau siapa yang patut menanggung siapa, maka perkahwinan seperti itu tidak ada masalah untuk diamalkan.

``Jika pihak isteri bersetuju bahawa lelaki tidak perlu memberi nafkah zahir seperti menyediakan rumah, pakaian dan sebagainya, tetapi hanya bertanggungjawab terhadap nafkah batin saja, maka ia tidak menjadi masalah.

Bagaimanapun, katanya perkahwinan ini harus dilakukan secara jujur dan tidak disalahgunakan bagi menganiaya kaum wanita.

Kejohanan Sukan Antara Fakulti Perubatan IPTA di UPM



I took part in this tournament over the weekend in UPM. It had been quite some time since i last joined any sports carnival. It really felt good being young again.. heks.. not that i am that old but i had my share of fun. The Opening ceremony started late on Friday morning ; we were so damn HOT under the sun when the speeches were given. And to top it up..we had a half an hour session of "senamrobik" !! Unfortunately non of the big bosses came in the morning , so it was me and Zaqrul being the only doctors from the faculty around.. hence Zaqrul the "Dean" and me the "Deputy" !!! haks..

Perbarisan..


I was involved in Volleyball and 9 a side footie. I was scheduled to play badminton as well but it clashed with my volleyball.

Obviously , being unprepared ; we were punished by the other team especially in football. Haks... i did give my best until my legs went crampin' like shit !!! haks.. In volleyball , we lost narrowly to the champions and we played a very good game. Luckily , my spikers were athletic thus able to spike the ball down. But digging was out and we lost mainly because of that. I am happy though as we thoroughly enjoyed ourselves. Seeing the UM fellas cam cuak.. heks..

Volleyball team..cum football team..cum badminton team.. cum..etc..


UiTM will be the host next year so watch out for us next year !!!

I missed this yesterday !!!! Mak , Wifey and baby Dzaeff went to this place ; somewhere in Pandan Indah as one of her friend organized a birthday party there. It's a water theme park ; on top of a shopping complex in Pandan Perdana..

And where was I ? I had to sit for my IELTS exams in British Council.. hmmmpphhh..
I certainly missed the fun of seeing baby Dzaeff enjoying himself in the water. I would love to bring him again to SUnway Lagoon perhaps next time around ? haks.. A vision for him to start swimming early so that he'll do incredible wonders. ( Dapat Dato' ke macam Malik Mydin.. haks.. )

A tribute to my sifu

It was teacher's day yesterday ; I was on my way back home from a luncheon with Dil when i suddenly decide to go back to my old school. I had about 2 hours before i can pick Dzaeffran up so why not spend some time with an "old" friend !!

Haks.. Met up with Ustaz Ahmad yesterday . The last time i met him was during the annual Qurban early this year but it had been quite some time since we sat and talk.
He was still his old self ,only that i reckon he has toned down a lot having teaching in the school for more than 16 years.

I have this special relationship with him since he first came to SMTTDI in 1990.He was a young 21 year old then ; rebellious and raw. I never saw him as a "master-force" , but we were more of a mentor-mentee thingy rather than the typical Ustaz teaching us religious studies.He was close to me that everybody in school know how we walk in each other's shadow. He was a big influence to me during my school days ; and i am glad that he did that . I reckon a lot of my colleagues would agree that he exerted certain influence and idealism to our school . I am certain that we all will remember his "ways" of things .. Ask Zain of Ruffedge or Shah ,Lah and Zaf of VE.. Ustaz Ahmad's name mention still make their spine to shiver !!!

I may not be a pious scholar but there are certain values he taught me that i hold on to till now. He was a very enthusiastic teacher ; not just teaching Islam but about life as well.HIs encouragement certainly motivated me to strive and BE the best. His experience sharing was certainly helpful and had shaped my way of thinking .

And today , he is still as active as he was in the yesteryears. He is currently planning for his 7th trip to Gunung Tahan next month ! We laughed yesterday remembering the first ever trip to Gunung Tahan in 1992 ! It was a very memorable one and he still remembered our group. Not just because we were his first mountain climbing group , but of the famous "nasi kepal " incident which taught us all a good lesson !!

Selamat Hari Guru buat semua guru-guru yang ikhlas mendidik anak bangsa...

I was the presenter for today's CME . I'm always keen on presenting stuffs so my preparation was thorough so that i can answer my specialist's KILLER questions. heks.. It was an enjoyable session talking about steroids.

I'm on leave for the whole of next week. Somehow , I'm beginning to feel sad leaving Selayang. The Anaesthesia Department is really cool and i love working with them. The specialists are really nice and fun to learn from. Its a very family-like atmosphere and i enjoy it very much.Its a pity that i have to go to UM soon and stay there for four years.

Hopefully , when i return - it will still be as enjoyable as it is now..
